Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical and dental care in five counties in northwest Ohio including: Henry, Seneca, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and is based in Fremont, Ohio. CHS physicians, dentists and nurse practitioners provide a broad spectrum of care in specialties such as Family Practice, Pediatrics, Internal Medicine, Obstetrics, Gynecology, Behavioral Health and Dentistry.
CHS is accredited by the Joint Commission and is certified as a “Primary Care Medical Home” or PCMH. This special accreditation means that CHS is recognized as a model for primary care excellence that is patient-centered, comprehensive, coordinated, accessible, and focused on quality and safety. Patients are treated with respect, dignity and compassion.
Over the years, CHS has earned the reputation as a leader for excellence in the region and has a proven track record of meeting or exceeding standards set for the health care industry.
