Team Select Home Care was established in 2008 with the goal of bringing together the most experienced and ethical home health professionals in the industry.

Our unique Medicare focus has allowed us to develop specialized programs and 'Patient Kits' which serve to ensure reduced rates of rehospitalization, and increased recovery and overall health.
