The Case manager conducts screening of new clients following the guidelines of the agency and agency approved procedure, which ensures that all individuals are adequately, and appropriately served according to their individual needs and in accordance with existing agency policies and procedures. The Case manager also accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessment on clients requesting services, entering caseload and develops appropriate treatment/service plans with clients in order to provide appropriate services and develop professional relationship with clients. The Case manager also provides on-going supportive and/or case management services in accordance with the problems, needs, and the strategies identified within the service plan in order to help the clients to achieve the stated goals and objectives.
