Purpose:
Facilitate, develop, monitor, measure, and report on health, wellness, and condition management programs that complement the Health and Wellness Clinic maintained and coordinated by Healthstat, Inc. The Health & Wellness Coach will present information about how to improve health, prevent disease, and modify health behaviors to individual patients through one-on-one health & wellness coaching, as well as to client groups. In addition, the Health & Wellness Coach will serve to coordinate efforts among all client vendors to provide cohesiveness of services and to better engage the client population. He or she will also be responsible for creating and writing educational materials, conducting meetings, and planning informative, interesting, and effective programs. The Health & Wellness Coach Supervisor will supervise all Wellness Coordinators assigned to the client. These duties are carried out for a specific client at its various locations.
