Overview

A Care Manager provides services similar to case management, with additional teaming to support client needs

• Provide initial assessment and help develop a plan of care
• Oversee client care and monitor progress
• Connect families with coordination of services, making referrals, and linking them with resources
• Coordinate with providers on a regular basis based on client needs

The right person will enjoy working as part of a team, working collaboratively to meet the individualized client's needs.

• Education: Bachelor’s Degree in related field
• Certification: MHRT/C
• Experience: Five years of human service preferred
• Reliable transportation and valid driver’s license
